Inspector Notes
An unannounced renewal inspection was conducted on 11/28/2022 from 8:20am to 10:45am. There were 10 children ages 4 months old to age 3 years old (29 points) with the Provider and 1 assistant. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, 10 children?s records, emergency drills, and emergency supplies were reviewed. Children were observed participating in using the swing, building, dramatic play, table toys and snack. There was an adequate number of staff present with current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Violations
9Evidence: The record for Caregiver #2 did not contain a report documenting the absence of Tuberculosis.
Evidence:
1)The seat of the high chair was torn in a number of places and the non-absorbent stuffing inside of the high chair seat was not waterproof or able to be sanitized.
2)There was a broken component on the play equipment outdoors that was hanging from the structure and had a sharp fastener exposed that could snag a child?s skin.
Evidence: There was no documentation on file that the cat belonging to household member #4 was current with rabies vaccination. The rabies vaccination on file expired on 8/16/21.
Evidence: The sheets in the crib were dirty, spotted and discolored where fluids had dried on the sheets.
Evidence: During the inspection, Child #8 (date of birth 7/28/22), was placed in a room to sleep alone with the door closed at approximately 9:00am and was not checked on until approximately 9:40am. There was no baby monitor in the room.
Evidence: Upon arrival for the inspection at approximately 8:20am, the Provider was alone with 9 children (25 points) and Caregiver #2 did not arrive until approximately 9:06am.
Evidence: During the inspection, Child #2, Child #3, Child #4, Child #5, Child #6, Child #9 and Child #10 sat down to eat a snack and none of the children washed their hands prior to eating.
Evidence: The provider fed Child #7 (DOB 4/13/22) a bottle while they were laying in a bouncy seat. Child #7 was not strapped in during the feeding.
Evidence: The bottles for Child #7 were not dated as required.
