Based on review of 3 Household member records, the provider did not obtain a sworn disclosure, Central Registry check or Criminal History check every five years from the date of the last check.

Evidence:
1)The record for Household member #2 did not contain documentation that a sworn disclosure had been completed every five years from the date of the last sworn disclosure. The last sworn disclosure was completed 2/10/18.
2)The record for Household member #3 did not contain documentation that a Central Registry check or Criminal History check had been completed every five years from the date of the last checks. The last Central Registry check was completed on 11/28/17. The last Criminal History check was completed on 12/4/17.

Based on observation, the Provider did not ensure that areas and furnishings of the family day home, inside and outside were maintained in a clean, safe and operable condition.

Evidence:
1)On the hard plastic play structure outside, there is a broken tunnel piece that is flopped open and 2 screwheads are protruding an inch which could cause it to snag a child?s skin.
2)On the swingset, there is a screw protruding 7 threads which could cause it to snag a child?s skin.

Based on observation, the provider did not ensure that potentially poisonous substances were stored in an area inaccessible to children.
Evidence:
1)There was sanitizer that was accessible to children hanging on the changing table edge in the children?s play area.
2)There was a container of joint compound that was sitting on the floor in a closet in the kitchen that was unlocked and accessible to children.

Based on observation, the provider did not ensure that sharp kitchen utensils and other sharp objects were inaccessible to children unless being used or with children under close supervision.

Evidence: There was a sharp knife that was stored in a drawer in the basement kitchen where children have access.

Based on observation, the provider did not cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers.

Evidence:
1)In the children?s play space in the basement, there was an unused outlet that was not covered.

2)In the children?s napping room in the basement, there was an unused outlet that was not covered.

Description: Based on review of the written attendance record and interview with the Provider, the Provider did not ensure they maintained a written record of children in attendance each day.

Evidence:
During the inspection in was determined there were 9 children present, but the written record reflected there were no children present.

Based on observation, the Provider did not ensure that outdoor play equipment has ?S? hooks that are tightly closed and that swings have flexible seats of rubber, canvas or nylon.

Evidence:
1)On the plastic swing set, the ?S? hooks on the swings were not tightly closed.
2)On the plastic swing set, the swing seats were hard plastic and were not flexible.

Based on observation, the provider did not ensure that a safety strap was used and securely fastened in an infant seat that was occupied.

Evidence: During inspection, an infant was placed in a high chair where the safety straps were not fastened.

Based on observation, the Provider did not ensure that topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine and insect repellent were inaccessible to children.

Evidence: There were 12 diaper creams stored on one of the lower shelves on the changing table in the children?s play space where children have access.
