Inspector Notes
An unannounced monitoring inspection was conducted on 05/25/2023 from 10:25am to 12:00pm. There were 9 children ages 9 months to age 4 years old (24 points) with the Provider and 1 assistant. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, 9 children?s records, emergency drills, and emergency supplies. Children were observed watching television and transitioning to lunch. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in MAT, CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Violations
9Evidence:
1)The record for Household member #2 did not contain documentation that a sworn disclosure had been completed every five years from the date of the last sworn disclosure. The last sworn disclosure was completed 2/10/18.
2)The record for Household member #3 did not contain documentation that a Central Registry check or Criminal History check had been completed every five years from the date of the last checks. The last Central Registry check was completed on 11/28/17. The last Criminal History check was completed on 12/4/17.
Evidence:
1)On the hard plastic play structure outside, there is a broken tunnel piece that is flopped open and 2 screwheads are protruding an inch which could cause it to snag a child?s skin.
2)On the swingset, there is a screw protruding 7 threads which could cause it to snag a child?s skin.
Evidence:
1)There was sanitizer that was accessible to children hanging on the changing table edge in the children?s play area.
2)There was a container of joint compound that was sitting on the floor in a closet in the kitchen that was unlocked and accessible to children.
Evidence: There was a sharp knife that was stored in a drawer in the basement kitchen where children have access.
Evidence:
1)In the children?s play space in the basement, there was an unused outlet that was not covered.
2)In the children?s napping room in the basement, there was an unused outlet that was not covered.
Evidence:
During the inspection in was determined there were 9 children present, but the written record reflected there were no children present.
Evidence:
1)On the plastic swing set, the ?S? hooks on the swings were not tightly closed.
2)On the plastic swing set, the swing seats were hard plastic and were not flexible.
Evidence: During inspection, an infant was placed in a high chair where the safety straps were not fastened.
Evidence: There were 12 diaper creams stored on one of the lower shelves on the changing table in the children?s play space where children have access.
