Inspector Notes
An unannounced complaint inspection was initiated on 08/21/2023 from 12:45pm to 1:45pm as a result of a complaint received in our office regarding supervision during nap time. There were 6 children present ages 8 months to age 3 years with the Provider and an assistant. All of the children were laying down for nap time at the time of inspection. The program area, attendance, 3 child records, 3 staff records, and documentation were reviewed. The complaint was determined to be invalid, however there were 7 violations cited unrelated to the complaint. The Areas of non-compliance are identified in the Violation Notice.

Violations
7Evidence: The record for Caregiver #3 (Date of Hire 8/21/23), did not contain documentation of a fingerprint background check completed prior to the first day of employment.
Evidence: The record for the Provider did not contain documentation that a criminal history background check report had been completed every five years from the date of the last check. The last criminal check was completed 6/20/18.
Evidence: The record for Caregiver #3 (date of hire 8/21/23) did not contain documentation of a completed sworn disclosure statement prior to the date of hire.
Evidence: There was no file for Caregiver #3 (Date of hire 8/21/23).
Evidence: The Provider does not have documentation of a current certification in CPR or First aid. The certification on file for CPR and first aid expired on 7/31/23.
Evidence:
During the inspection in was determined there were 6 children present, but the written record reflected there were 4 children present.
Evidence:
1)Upon arrival to the home, blankets were observed draped over the side of 2 of the infant cribs where infants were resting.
2)A stuffed animal was observed in the crib of one of the infants while they were sleeping in the crib.
