Inspector Notes

An unannounced monitoring inspection was conducted July 13, 2023 from 3:15 pm to 3:45pm. There was 1 child for a total of 2 points directly supervised by 1 staff. The home, 2 staff records, 1 child 's record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an assortment of age appropriate materials for children. The home was clean and organized.

Based on review of two staff records, the center did not update criminal history record reports every five years.
Evidence:
1. Staff #1 criminal history record check expired on 6/05/2023.
2. Staff #2 criminal history record check expired on 7/13/2023.
Plan of Correction: We will get fingerprints as soon as possible.

Based on children records review, the provider did not obtain at least two emergency contact information.
Evidence:
1. Child #1 (date of enrollment 10/15/2018) did not have at two designated persons to contact in case of emergency if the parent cannot be reached.
Plan of Correction: Will ask parent for emergency contact.
