Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/23/2024 and completed on 10/11/2024, as a part of the licensure period. The on-site inspection began at 2:40pm and ended at 4:55pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 4 staff. The inspector reviewed 5 children?s records on-site and 2 staff records electronically on 9/30/2024. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/18/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside are maintained in a clean, safe and operable condition.

On the playground, a corner area of concrete had broken off the cement court. Wooden timbers were positioned next to the concrete and created a tripping hazard for children. A child was observed tripping over the timbers and falling.
Plan of Correction: The corner area has been fixed.

Systemic Violation
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Child 6, 7, 8, and 9 were observed in care during the inspection on 9/23/2024. All of the children were school-age and are enrolled in another center. There were no records available on site for any of the children that included health information. One child was missing complete parent contact information and emergency contact information. The children have been coming to the center for care for approximately one month.
Plan of Correction: We have a few children that Aquatic picks up from school and is transferred over to our 460 locations after all schools arrive due to siblings attending other location. These children do know the teachers as they work at both locations. Moving forward we will make sure that they have a copy of their records on site instead of just using procare
Standard 8VAC20-780-340-F
Children under 10 years old shall always be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.

Child 5 (age 8) was observed in a restroom area without supervision for approximately 3 minutes. The restroom is located on the opposite side of the building from the child's classroom. There were no staff within sight or sound supervision.The child had unsupervised access to this area. There were 4 school age children present from another location at the center. The children were in care for approximately 35 minutes. These children were not within actual sight and sound supervision of staff from their center.
Plan of Correction: CD has discussed with staff and training will be provided by our PD for the restrooms. The children that were there from the other site were in sight and the sound of a teacher just was one of the Aquatic teachers.

Systemic Violation
Centers shall be in compliance with the center's own policies and procedures required by these standards.

The center?s written policy is that children shall remain under supervision of staff at all times. On 9/23/2024, Child 5 (age 8) was left unsupervised in a restroom area on the opposite side of the building from their classroom for approximately 3 minutes.
Plan of Correction: CD has discussed with staff and training will be provided by our PD.
Standard 8VAC20-780-70
Systemic Violation
Staff records shall have the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center.

Staff 2, who has been actively working at the center for 2 weeks, did not have emergency information at the center at the time of the inspection.
Plan of Correction: The staff had in emergency contact in file however, just had started and the file was still at the office.
Standard 8VAC20-780-80-A
Systemic Violation
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Two groups of children were on the playground, one group had a total of 20 children with one staff, the other had 4 children with one staff. There were no attendance records with the staff in each group. The staff with 20 children was unable to tell the inspector the correct number of children they had in care. The children had to be stopped and counted.
Plan of Correction: The children had just arrived at center and the coordinator had records of the children in attendance did not take outside and update the teacher how many they had. This will be discussed with them for follow up.
