Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on 1/10/2022 and concluded on 01/13/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 10 children present, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on record review and interview with the provider, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The records for Child #1 (start date 10/20/2021) and Child #2 (start date 09/07/2021) did not contain documentation of a physical examination report.
2) The provider could not locate the physical examination report in the records for Child #1 and #2.
Plan of Correction: The provider shall contact the parents of Child #1 and Child #2 and request a physical examination report.
