Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 07/06/2022. There were 9 children present with 3 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on documentation review, the provider did not maintain a written hard copy record of daily attendance that accurately documents the arrival and departure of each child in care as it occurs.
Evidence:
1) The record of attendance for the dates of June 21, June 30, July 1 and July 5 did not document the arrival or the departure of children in a consistent way: The arrival of the child would be documented but not the departure or vice versa.

Based on record review, a written care plan was not available for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1) On the date of inspection, the record for Child #1 did not contain a written care plan. The health exam for Child #1 documents a food allergy and emergency medication is available at the fdh.
