During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 01/23/2025, as a part of the licensure period.

The on-site inspection began at 11:45 am and ended at 1:10 pm. The inspector reviewed compliance in the areas listed above.

There were 7 children present and 3 caregivers; the point total was 22. The inspector reviewed 2 children?s records and 5 caregiver/household members records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 02/04/2025. A POC submitted after this date will not appear on the public website.

The provider must obtain documentation that the substitute provider have the required qualifications.

There was no documentation of Staff #4's qualifications.

A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.

There was no record of the times that the substitute provider has worked.

A tuberculosis (TB) screening is required for each caregiver every two years from the last screening.

An updated TB screening for Caregiver #1 was not completed and was overdue by one month.

Wood burning stoves and fireplaces and associated chimneys shall be inspected annually.

The most recent fireplace inspection was overdue by 20 months.

Children's information shall be privileged and confidential.

Several Supplemental pages containing confidential information on children were posted in the fdh.
Plan of Correction: Completed the information the same day of the inspection.

Long-term medications shall be given to a child only with written authorization from the child's physician and parent.

There was no authorization form for medication for Child #2, Child #3, Child #4, Child #5 and Child #6.
