Inspector Notes
An announced inspection was completed on 07/25/2022 in response to a planned relocation of the licensed program. Any areas of noncompliance are documented in the violation notice. If there are any questions, please contact karen.dickens@doe.virginia.gov.
Violations
8Evidence:
1. The family day home was not equipped with at least one carbon monoxide detector.
2. Staff #1 stated that a carbon monoxide detector was purchased, but could not locate it during the time of the inspection.
Evidence:
1. Two protective covers were missing from electrical outlets.
Evidence:
1. The cribs were not labeled for individual use. Another child was placed in the crib that was verbally identified as Child #1's crib.
Evidence:
1. Child #1 fell asleep in a swing at 10:50am. Child #1 was not removed from the swing until 11:10am.
Evidence:
1. Child #1's record (Start date-07/11/22) did not contain documentation of a parent's work address and the medical insurance policy number.
Evidence:
1. Child #2's record (Start date-01/04/21) did not contain documentation of additional immunizations. The last immunization was received on 05/25/21.
Evidence:
1. The posted menu did not contain dates for each week of the menu.
Evidence:
1. A total of six bottles in the refrigerated did not contain the children's full name and date.
