Based on observation and interview, the family day home was not equipped with at least one carbon monoxide detector.
Evidence:
1. The family day home was not equipped with at least one carbon monoxide detector.
2. Staff #1 stated that a carbon monoxide detector was purchased, but could not locate it during the time of the inspection.

Standard 8VAC20-800-320-B
Based on observation, protective covers were not installed on all unused electrical outlets.
Evidence:
1. Two protective covers were missing from electrical outlets.
Plan of Correction: Protective covers will be placed on the electrical outlets.

Based on observation, a child was not provided an individual crib.
Evidence:
1. The cribs were not labeled for individual use. Another child was placed in the crib that was verbally identified as Child #1's crib.
Plan of Correction: Cribs and cots will be labeled for individual use.
Standard 8VAC20-800-590-B
Based on observation, an infant who falls asleep in a play space other than his own crib was not moved promptly to his designated sleeping space.
Evidence:
1. Child #1 fell asleep in a swing at 10:50am. Child #1 was not removed from the swing until 11:10am.
Plan of Correction: Moving forward, when infants fall asleep in anything other than their cribs, they will be moved to their cribs promptly.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-07/11/22) did not contain documentation of a parent's work address and the medical insurance policy number.
Plan of Correction: I will have the parents to provide the missing information and document the record.

Based on record review, the family day home did not obtain documentation of additional immunizations once every six months for a child under the age of two years.
Evidence:
1. Child #2's record (Start date-01/04/21) did not contain documentation of additional immunizations. The last immunization was received on 05/25/21.
