Inspector Notes

An unannounced renewal inspection was conducted today between the hours of 10:00 am through 11:00 am. There were a total of four children (13 points) observed with one caregiver. A total of four children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, hand washing, and other organized activities. Based on record review, children's records did not contain required information.

Evidence:
1. Child #1's record (Start date-11/7/21) did not contain the parent's work address and phone number, the address for a second emergency contact, and the medical insurance and policy number.
Plan of Correction: The provider will get the information from the parents and document the record.
