Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:30 am through 11:30am. There were a total of nine children (25 points) observed with three caregivers. A total of five children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, singing and dancing, snack, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. 1. In the bathroom, cleaning agents and disinfectants were stored in a low cabinet that was not locked.
Evidence:
1. Child #2's record (Start date05/22/2023) did not contain two parent work addresses, the medical insurance policy number, and written authorization for emergency medical care should an emergency occur and parent cannot be located immediately.
2. Child #4's record (Start date-01/27/2023) did not contain the name of medical insurance and policy number.
