Violations

2
Standard 22.1-289.035-A
Based on record review and staff interview the center did not ensure to obtain background checks every five years.

Evidence: The director confirmed the most recent criminal history check (3/7/2018), central registry check (8/6/2018) and the sworn statement (12/22/2017) for staff #4 were more than 5 years old.
Plan of Correction: All background checks were done on 4/3/2024 and are awaiting results.
Standard 8VAC20-780-80-A
Based on observation and staff interviews the center did not ensure for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: Staff in the following rooms confirmed their attendance sheet did not show the accurate number of children present:
1. The infant and toddler room had 8 children present and only 7 were checked in.
2. The 3 year old room had 6 children present and only 5 were checked in.
Plan of Correction: Reminded the staff on how to use the sheet and to complete periodic checks on the attendance just in case we had a late admittance in the classroom.
