Inspector Notes
An unannounced monitoring inspection was conducted July 8, 2024 between the hours of 9:40am - 11:00am. There were 9 children(29 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 1 household member record, 5 children?s records, evacuation drills, and policies were inspected. Children were observed playing with the caregivers. There was an assortment of age-appropriate materials for children. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website. Evidence:
1. Household member #4's central registry search results were dated 2/17/2019. (expired 2/17/2024)
Based on record review, child records did not contain all required information.
Evidence:
1. The records for Child #1 (date of enrollment: 3/01/2024) and Child #3 (date of enrollment: 4/15/2024) did not contain written authorization for emergency medical care.
Based on record review and interview, the family day home did not ensure that all emergency evacuation drills were documented.
Evidence:
1. The last documented emergency evacuation drill was dated 5/15/2024.
2. Caregiver # 1 acknowledged that the emergency evacuation drill for June 2024 was not documented.
Evidence:
1. Child #2 (18 months) most recent documented immunizations were dated 5/25/2023.
