Inspector Notes
A subsidy health and safety inspection was conducted on 08/17/2022. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the vendor.

Based on record review, the vendor failed to ensure that staff have Current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

The CPR certification for staff 1 expired on 03/27/2022 according to written documentation on file at the facility.

Plan of Correction: Administrative staff stated that staff 1 had completed an updated CPR and First Aid class however, the certificate/card could not be located. If the card can not be located the instructor will be asked to provide a new card and/ or staff 1 will complete a new class.
