Inspector Notes
An unannounced inspection was initiated on February 14, 2025 and completed on May 2, 2025 in response to a complaint received by the licensing office on January 27, 2025 relating to allegations of forbidden actions and supervision. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/20/2025. A POC submitted after this date will not appear on the public website.
Violations
2Preventative Measures:
?The classroom routine schedule has been revised and posted to reflect supervised transitions.
?When staffing allows, a two-staff protocol is used during bathroom/diapering times to ensure uninterrupted supervision.
Ongoing Monitoring:
The director will conduct unannounced walkthroughs during diapering and toileting times to ensure ongoing compliance. Non-compliance will result in immediate retraining or corrective action.
Staff Accountability:
All current staff have signed an acknowledgment form confirming their understanding of the regulation and the expectations for supervision at all times. These forms are maintained in employee files.
The specific highchair in question has been immediately and permanently removed from the premises. All staff have been reminded that no equipment or furniture shall be used for purposes other than its intended and approved use.
Preventative Measures:
?A mandatory staff meeting and retraining session was held to reinforce appropriate use of furniture and the prohibition of any form of confinement not allowed under regulation.
?A visual reminder has been posted in each classroom as reinforcement.
?New hire orientation now includes review and signed acknowledgment of this regulation.
Ongoing Monitoring:
The director will conduct bi-weekly classroom audits to ensure compliance, with on-the-spot coaching and documentation as needed.
