Areas Reviewed

A monitoring inspection was conducted on 10/18/2022 between the hours of approximately 09:30 am and 10:30 am. There were 32 children present, ranging in ages from 3 years to 5 years, with 4 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-40-M
Based on documentation review, allergy lists did not contain all required information.
Evidence:
1) Allergy lists were not dated as required.
2) Allergy lists in each class did not include the list of children with allergies in the other classroom.
Plan of Correction: We will post both lists in both rooms and date the list.
Standard 8VAC20-780-420-E-3
Based on record review, the center did not request, at least annually, parent confirmation that the required information in the child's record is up to date.
Evidence:
1) The most recent review of the information in the record for Child #2 was dated 09/01/2021; and for Child #4 was dated 08/05/2021.
Plan of Correction: Will document annual review of information.
Standard 8VAC20-780-50-A
Based on observation, the center did not keep children's information confidential.
Evidence:
1) The "Supplemental Page" of the previous licensing inspection containing confidential information about children in care was posted on the center's information bulletin board.
Plan of Correction: Was removed.
Standard 8VAC20-780-60-A
Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain work phone number and place of employment of each parent who has custody, and the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
2) The record for Child #3 did no contain the addresses of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: Enrollment coordinator will reach out to families to get the information.
Standard 8VAC20-780-70
Based on record review, staff's records did not contain all required information.
Evidence:
1) The records for Staff #1 and Staff #2 did not contain verification of name, address and age. (I.D.)
