During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 05/12/2025, as a part of the licensure period, The on-site inspection began at 3:30 PM and ended at 4:55 PM. The inspector reviewed compliance in the areas listed above. There were 19 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site.
This inspection included:
? document review,
? tour of the facility,
? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4The record provided for child 1 did not contain a record of immunizations.
The immunization records in the record provided for child 2 were not signed by a physician.
Children will not be allowed to attend without a copy of immunization records signed by a physician.
The record provided for child 2 did not contain the results of a physical examination.
All children's records will be reviewed to ensure that all children's records have the results of physical examinations.
A minor injury record from January 2025 for child 3 did not contain the time the parent was notified of the child's injury.
All written records of children's injuries will contain the required documentation.
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the parents place of employment and business telephone.
The record for child 3 contained the father's place of employ but no business telephone number.
Children's records will be reviewed to ensure that all required information is documented.
