Inspector Notes

A non-mandated inspection was initiated on March 4, 2022 and concluded on March 9, 2022. A self-reported incident was received in the Office of Child Care Health and Safety (Valley Office), on March 4, 2022, regarding forbidden actions. Interviews were conducted on March 9, 2022. One child record, one staff record, attendance record, and four staff interview were conducted. No violations regarding the self-reported allegation were cited.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A-1
Based on review of staff records, the center failed to ensure that tuberculosis screening/test were completed before staff come into contact with children.

Evidence:
1. Staff #1 tuberculosis screening was dated 03/02/2022. Date of hire for Staff #1 was 02/25/2022.
2. Staff verified that Staff #1 was at the program and working with children on 02/25/2022.
Plan of Correction: There has been a change in administration. During the interim this slipped through. All site managers have been retrained, and reviewed the requirements for tuberculosis screenings.

Based on review of staff records, the center failed to obtain two references before the employees first day of hire.

Evidence:
1. There was only one completed reference on file for Staff #1. Date of hire for Staff #1 was 02/25/2022.
2. Staff verified that there was only one completed reference on file, and the date of hire for Staff #1.
Plan of Correction: Administration immediately contacted another reference and received a written reference via email on 3/7/2022. All site managers were retrained and reviewed the requirements and timeframes for obtaining reference checks.
