Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on July 5, 2022 from 10:30 A.M.-12:00 P.M. on site, and from 12:20 P.M.-1:45 P.M. at the administration office for staff record review . There were 16 children present with three staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-600-D-1
Based on review of staff records, the center failed to ensure that all employees had certification in cardiopulmonary resuscitation (CPR) within 90 days of employment.

Evidence:
1. Staff #4's date of employment was 01/24/2022. The CPR certification was dated 05/21/2024. CPR certification was required by 04/24/2022.
2. Staff verified the date of employment for Staff #4 and the date on the CPR certification card.
Plan of Correction: The certification has been completed an din the future deadlines will be established for new staff to complete the required training on time.

Based on review of staff records, the center failed to ensure that all employees had certification in first aid within 90 days of employment.

1. Staff #4's date of employment was 01/24/2022. The first aid certification was dated 05/21/2024. First aid certification was required by 04/24/2022.
