Standard 8VAC20-780-130-A
Repeat Violation
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1,who had been attending for three months, did not have documentation of an immunization record on file.
Plan of Correction: A new enrollment procedure has been developed for new enrollees that requires submission of the physical before the student may attend versus requesting it form the schools.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, who had been enrolled for three months.
Plan of Correction: A new enrollment procedure has been developed for new enrollees that requires submission of the physical before the student may attend versus requesting it form the schools.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. Child #2, child #3, child #4, and child #5, who have been attending the center for three months, each have a diagnosed food allergy.
2. Staff #1 was not aware of any of these allergies.
3. Staff #2 was not aware of three of the allergies.
4. Child #2 has a plan from the parent to administer a medication if needed for the allergy but the center did not have any medication on file for the child.
5. Child #5 requires the use of an emergency medication for the food allergy.

Repeat Violation
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and that shall contain all required documentation. An injury record for #7 did not contain how the parent was notified.
Plan of Correction: A reminder was sent via email to all LGPS staff regarding the required documentation on injury forms. Site managers will review forms regularly to ensure completion.

Repeat Violation
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information.
1. The record for Child #3, enrolled for three months, was missing two emergency contacts.
2. The record for Child #4, enrolled for three months, was missing the address of one emergency contact.
3. The record for Child #5, enrolled for three months, was missing a second emergency contact person.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #2, child #3, child #4, and child #5, who have been enrolled for three months, each have a diagnosed food allergy and did not have a written care plan in their files.
