The center shall inform all staff who work with children of children?s allergies, sensitivities, and dietary restrictions.

Plan of Correction: The site manager will ensure all staff are aware when enrollees enter the program and have a food allergy. Staff will be quizzed regularly on which students have food allergies to ensure everyone is up to date. This may be done by the site manager or operation manager during their site visits.

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
1. Child #5, diagnosed with a food allergy, had been in care over a month, and the list, dated 08/19/25 had not been updated to reflect the child?s diagnosed food allergy for which the child is prescribed emergency medications.
2. The list in the after-school program was dated 8/19/25 and contained information for child #1 and child #2, who left the program in December 2025.

Plan of Correction: The list of food allergies will be updated to include current enrollees and remove those no longer attending, initialed and dated. The site manager had been instructed that they will need to ensure this is done as students enter and exit the program.

The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the medication is administered by a staff member or an independent contractor who has completed Medication Administration Training (MAT).

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
1. The record of child #5, enrolled for five weeks, was missing the following required components: first date of attendance, documentation of immunizations, and documentation of a physical exam.
2. The records of child #1 and child #2, enrolled for five months and no longer attending, did not contain the last date of attendance.

Plan of Correction: The Operations Manager will review all new contract requests for new enrollees with the site manager prior to approving contracts to ensure all required paperwork is received.
