Inspector Notes

An unannounced monitoring inspection was conducted on 3/30/2023 and concluded with an electronic record review on 04/03/2023. There were 28 children, ages 3 years- 5 years, and 5 staff providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in mealtime, teacher directed activities, and self-directed activities. The inspector discussed the following with provider: allergy list, and playground. A total of 5 children?s records, and 5 staff records were reviewed. The inspector arrived for the inspection at 9:15 am and departed at 10:45 am.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and discussion with staff, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: As of approximately 9:15 am on 03/30/2023, the daily attendance for the group of children in Class 16 had not been taken. Staff 1 stated that they usually take attendance after the bus arrives with the majority of the children. There was a total of 6 children in care. The daily attendance record did not document the arrival of each child as it occurred.
Plan of Correction: Staff will take attendance at time of arrival and departure.
