During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/09/2025 and completed on 9/22/2025. The on-site inspection began at 10:12 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/29/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #2, employed for over three weeks, did not have documentation of requesting a central registry check and sex offender name check from two states. Staff #2 has worked alone with children.
Plan of Correction: We will get the needed background checks immediately. Staff will not be left alone in the future without the background checks.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

A dated allergy list was not available in each room or area where children are present.
Plan of Correction: We have added the list to each room.
Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff #4 did not have a current cpr and first aid certification. Administration stated that Staff #4 supervises children alone during snack time.
Plan of Correction: We have 1 staff in each classroom and all others signed up.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #1, who has been enrolled for one week, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: We have sent home the needed plan and expect to get it back next week.
