Inspector Notes
A complaint investigation was initiated on July 16, 2021 and concluded on August 16, 2021. The complaint consisted of allegations regarding Programs (410-3). Based on interviews, a preponderance of evidence supports the allegations regarding forbidden actions. Therefore, the complaint is determined to be valid. Areas of non-compliance are identified in the Violation Notice.

Based on interviews, the center failed to ensure that staff did not engage in a forbidden action of punishment by another child.
Evidence: Interviews revealed that on multiple occasions between approximately January 2020 and May 2021 staff requested that Child #1 (age 11) leave the school age classroom to go to the 4 year old classroom to discipline their younger sibling, Child #2 (age 5). When Child #2 exhibited behavioral issues, Child #1 was instructed by staff to talk to the sibling or take the sibling into the bathroom. Child #1 stated that staff told them to scare Child #2, but not hit them and other staff told them to take the sibling into the bathroom and do what they have to do. Child #1 stated that they would take Child #2 into the bathroom to pinch them and Child #2 would cry.

Plan of Correction: Remind staff that children are not to correct behaviors of other children. Monitor staff to ensure children's safety.
