Inspector Notes

A complaint investigation was initiated on 8/3/2022 and concluded on 9/9/2022. The complaint consisted of allegations received by the Northern Licensing Office on 8/2/2022 regarding care of children, serious injury, supervision, and forbidden actions. Based on staff interviews and review of documentation, a preponderance of evidence supports the allegations regarding care of children and supervision. Therefore, the complaint is determined to be valid. Areas of non-compliance are identified in the Violation Notice.

Based on interviews, the center did not ensure that when staff are supervising children, they always ensure their care, protection, and guidance.

Evidence: On approximately Thursday 7/28/2022 or Friday 7/29/2022, Staff #3 and Staff #4 were ?sampling alcoholic beverages? in the center office while children were in care. Staff #4 reportedly put some alcoholic beverage into a Sprite can and took it into the Infant room. Staff #4 then stated ?I need to pour this out, I?m a light weight.?
Plan of Correction: There was no drinking while on the clock supervising children

Based on interviews, the center did not ensure that children under 10 years of age were always within sight and sound supervision of staff or that staff check on children who have not returned from the restroom after 5 minutes.

Evidence: On 8/17/2021, Child #1 (Age 6) was in the bathroom for approximately 10-12 minutes and was not checked on by staff. When Child #1 left the bathroom after approximately 10-12 minutes Staff #1 noticed that the child had sustained an injury to Child #1?s foot and there was ?blood everywhere? and the ceramic toilet lid was shattered.
