Inspector Notes
A Subsidy Health and Safety inspection was conducted beginning 8/3/2022 and concluded 9/9/2022 to review supplemental health and safety requirements. A licensing complaint inspection was also conducted this date. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on record review and staff interviews, the center did not inform the department?s representative as soon as practicable, but not more than two business days, of an injury to a child while under the center?s supervision when a referral is made for treatment from a medical professional.
Evidence: Child #1 (Age 6) sustained an injury to their foot on 8/17/2021 while using the bathroom. The injury report dated 8/17/2021 reads ?Child #1 went into the restroom to use it and when the child went to flush the toilet it did not flush so Child #1 picked the back part up and it slipped out of Child #1?s hand and fell on the floor and cut Child #1?s foot. The shattered pieces cut Child #1?s foot open.? Staff #2 called 911 and the child was taken to the hospital via ambulance and received several stitches. This incident was not reported to the department?s representative and was discovered during the course of a complaint investigation.

Plan of Correction: Any injury requiring medical attention will be called in to portal. Remind staff to check on children.
