Inspector Notes
An unannounced monitoring inspection was conducted 11/14/2022 from 10:45 to 11:45am to investigate a self-reported incident received by the Northern Licensing Office regarding the physical plant. Staff and child records were reviewed, the injury report was reviewed, and interviews were conducted. A violation was found regarding hazardous substances and is identified in the Violation Notice. Please provide a plan of correction for the noncompliance and include the date to be corrected. Please return a signed and dated copy to the licensing office by 5pm on 12/7/2022. Based on record review and staff interview, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children.
Evidence: On 11/4/2022, between approximately 5:30pm and 6:00pm, Staff #1 had approximately 8 children in the 4's classroom. Staff #1 had taken their keys, containing a cannister of pepper spray on an attached keychain, from the locked cabinet and sat them on a plastic tote bin by the door within reach of the children. Staff #1 was tending to another child and then they "heard the spray, turned and saw Child #1 with the pepper spray cannister and keys". Staff #1 immediately called management who began to administer first aid. Child #1 (Age 4) complained of burning hands and eyes. Staff also noted the child's hands appeared red.
