Inspector Notes
A complaint investigation was initiated with the center on 10/30/2023 and was conducted jointly with a local agency. The complaint consisted of allegations received by the Northern Licensing Office regarding supervision. Interviews were conducted and the center's own video recording was observed. The investigation was closed on 11/2/2023. There was sufficient evidence to substantiate the allegation regarding supervision and the complaint has been deemed valid. Areas of non-compliance are identified in the Violation Notice.

Violations
3Evidence: On October 25, 2023 Child #1 (Age 2) was left on the playground after the Threes room went back inside after their outside play time. Staff #1 took the other children in the Threes room back into the center at approximately 3:29pm while Child #1 remained on the playground. Child #1 ran to the front door of the center and began knocking on the glass approximately 30 seconds after the class entered the building. At approximately 3:30pm, a parent of another child at the center found Child #1 standing at the front door and rang the doorbell. Staff #2 answered the door at approximately 3:31pm and then escorted Child #1 to their classroom. Child #1 was out of sight and sound supervision for approximately two minutes.
Evidence: On 10/25/2023 from approximately 3:29pm to 3:31pm there were 10 children present with Staff #1. However, the youngest child in the Threes group at that time was Child #1 (Age 2) making the ratio 1 staff to 8 children.
Evidence:
1. Staff #1, Staff #2, and Staff #3 reported the center's policy for identifying where children are at all times requires staff to complete head counts at each threshold.
2. On 10/25/2023, Staff #1 did not not do head counts at each threshold when bringing in the Threes room from their afternoon outdoor play time and a child was left behind on the playground. Staff #1 stated "It was an accident, I forgot to do head counts."
