Inspector Notes
An unannounced inspection was initiated and concluded on 7/23/2024 in response to a complaint received by the licensing office on 7/8/2024 relating to allegations regarding administration. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
