During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 9/4/2024 and completed on 9/5/2024 in response to a complaint received by the licensing office on 8/27/2024 relating to allegations of ratio, supervision, physical plant, and behavioral guidance. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegations regarding ratio; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Allegations regarding supervision, physical plant and behavioral guidance were not able to be substantiated.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/30/2024. A POC submitted after this date will not appear on the public website.

The required ratio for children ages 16 months to 24 months is one staff per five children.

On 9/5/2024 at 7:00am there were two staff supervising 18 children in the Four's classroom. The youngest child in the group was age 21 months. The classroom had been out of ratio for approximately one hour.

The required ratio for children ages three years to school age eligible is one staff per 10 children.

On 9/5/2024 from approximately 7:13am through 7:43am there were 11 children with two staff in the Four's room. The youngest child in the classroom was age four years. The classroom was left out of ratio every few minutes during this timeframe as one staff from the Four's would go to receive children arriving to the program leaving 11 children with one staff.

Bottles shall not be used while the child in their designated sleeping location.

In the Infant room on 9/4/2024, Child #1 was observed drinking a bottle while laying in their crib which is their designated sleeping location.

The center shall maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

In the Two's classroom on 9/4/2024, there were 12 children present and only 11 children signed in on the attendance record.
