During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9/5/2024 and completed on 10/8/2024. The on-site inspection began at 7:00am and ended at 9:10am. The inspector reviewed compliance in the areas listed above. There were 30 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records electronically on 10/8/2024. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/30/2024. A POC submitted after this date will not appear on the public website.
Violations
6Staff #3, who had been employed for two months, did not have a central registry search request sent until more than five weeks of employment. The results were still not on file at the time of inspection.
Areas inside the center shall be maintained in a clean and safe condition.
1. Multiple areas of peeling blue wall paint were observed in the Four's classroom bathroom. Also, peeling wall paint was observed on the wall near the book corner in the Four's classroom.
2. In the Toddler's room, the bottom door on the changing table was broken exposing diaper creams to the children present in the classroom. The broken door was leaning on the wall nearby.
3. In the Two's classroom, the carpet wall siding had a three inch hole in the fabric and the top section of the siding was observed to be started to detach from the wall.
In the Infant room, there was one sunscreen that was not labeled with the child's name.
When diaper ointment or cream is used it shall be stored in a way that is inaccessible to children.
In the Toddler room, multiple diaper creams were stored in an open and accessible area underneath the changing table and 5 toddlers were present in the room at the time.
In the Toddler's room, there was a blue bottle containing milk that was not labeled or dated.
There were 11 children present in the Four's classroom but only two children were signed in on the written attendance record.
