During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and complete on 2/21/2025. The on-site inspection began at 9:10am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 31 children present and 7 staff. The inspector reviewed 5 children?s records and 8 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/3/2025. A POC submitted after this date will not appear on the public website.
Violations
9The center must request a search of the central registry prior to the employee's first day of employment.
Staff #2 and Staff #3, both employed for over four months, have not had a central registry search requested. Staff #2 was working alone with children at the time of inspection.
The Sworn Disclosure Statements maintained on site for Applicant #1 and Applicant #2 have been expired for one year and 10 months and 11 months, respectively.
Staff #2 and Staff #3 have not yet completed the VDOE sponsored orientation course and it is overdue by one month.
Areas inside the center shall be maintained in a clean, safe, and operable condition.
1. Multiple areas of peeling blue wall paint were observed in the Four's classroom bathroom.
2. In the Toddler room, the bottom door on the changing table was broken exposing diaper creams to the children present in the classroom. In addition, a section of cove base has detached from the wall by the exterior door exposing a layer of dirt and dust.
3. In the Twos room, there was an approximately four inch by two inch sized hole in the wall near the door.
Hazardous substances must be in a locked place that prevents access by children.
In the Threes classroom, there were three disinfectant spray bottles and two carpet cleaning spray bottles located on a shelf, out of the reach of children, in an unlocked cabinet.
In the Toddler room, Staff #2 did not wash their hands before or after changing a child's diaper.
When diaper ointment or cream is used it shall be stored in a way that is inaccessible to children.
In the Toddler room, multiple diaper creams were stored underneath the changing table in an open and accessible area and 6 toddlers were present in the room at the time. One toddler was observed reaching in and pulling out one of the tubes of diaper cream.
When food is brought from home, the container must be clearly dated.
In the Infant room, there were three bottles of breast milk that were not dated.
The center shall maintain a written record of daily attendance with each group of children that documents the arrival and departure of each child in care as it occurs.
In the Toddler Room, there were 6 children present and only 4 were signed in. In the Infant Room, there were 6 children present and only 5 were signed in.
