Inspector Notes
An unannounced monitoring inspection was conducted 3/14/2023 from 2:25 pm to 4:10 pm. There were 32 children in care, ages 5 to 12, in on group under the care of five staff. The Licensing Inspector reviewed five children?s records, one medication, and five staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. The children were observed having free play in the gym, homework, handwashing, and snack.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Based on record review and interview, the center failed to obtain the results of a Central Registry search for a new staff person by the end of the 30th day of employment.
EVIDENCE:
Staff 3 began employment on 5/23/2022. The Central Registry search results were obtained on 8/21/2022. Staff 6 stated they did not follow up with Central Registry to see why there was a delay in getting the results.
EVIDENCE:
Staff 5 began employment on 5/20/2022 and the is no documentation so support that the VDOE-sponsored orientation course (10 hour pre-service) has been completed.
EVIDENCE:
Authorization to administer Medication 1 to Child 1 expired on 2/14/2023, and the medication was still in the center on the day of the inspection.
