Inspector Notes
An unannounced review of staff records was initiated and completed on October 1, 2024, at the Wheeler Ave location. The review began at 11:00 am and ended at 2:45pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 2 staff records on-site. This inspection included document review, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statement for Staff #3, who is currently working, was dated four days after their first day of employment.
