Inspector Notes
A monitoring inspection was initiated and concluded on 3/1/2023. There were 4 daycare children and 1 household child present with the provider. The point total was 11 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 provider / household member records were reviewed. The inspection started at 12:55 pm and concluded at 3:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Based on a review of records and interview, the licensee was unable to demonstrate that all household members living in the home obtain the results of a search of the central registry within 30 days of turning 14 years old.
Evidence:
Four household records were reviewed. The record for household member #4, who is older than 14 years and one month, does not contain the results of a Search of the Central Registry.
The licensee acknowledged that there was an issue with he central registry request that was submitted and that the results were not available.
Evidence:
Four children's records were reviewed.
The record for child #4 did not contain the addressed for the 2 individuals designated as emergency contacts if the parents cannot be reached in an emergency.
The record for child #2 did not contain written information provided to and acknowledged by the parent regarding whether or not the family day home is covered by liability insurance.
The licensee was unable to locate this misising documentation during the inspection.
