Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 6/03/2024 from 2:00pm-3:40pm. At the time of entrance, 4 children, ages 2 to 3 years old,for a total of 8 points, were in care with the provider. The sample size consisted of 4 children?s records and 4 provider / household member records

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Based on a review of records and interview, the licensee did not complete the annual health and safety update course within 12 months of the last update course being completed.
Evidence:
The licensee's last documented Annual Health and Safety Training Course available for review was dated 12/15/2022. The licensee confirmed that she had not taken the health and safety update course within the last 12 months.
Plan of Correction: The licensee completed and sent the certificate for the annual health and safety update course the evening of the inspection on 6/3/2024.
