Inspector Notes

An unannounced, on-site renewal inspection was initiated completed on 02/24/2026, as a part of the licensure period. The on-site inspection began at 7:25 AM and ended at 8:21 AM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 1 staff. The inspector reviewed 5 children?s records on site and 4 staff records [on-site] OR electronically on March 2, 2026.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat violation
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
There was no documentation of immunizations for child 3 who began attending care on February 9, 2026 and was in care during the inspection.

Staff shall obtain a tuberculosis screening at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.

Repeat violation
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff 1 stated that she started working with the children in November 2025. There was no documentation that staff 1 had obtained the required orientation training. Staff 1 was alone with the children during the inspection.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.
Staff 1 was alone with the children from the center opening at 7:00 AM until another staff person arrived at 8:10 AM at which time there were seven children in care.
Staff 1 and staff 2 are regularly scheduled to care for the children in the before school program. There was no documentation to demonstrate that either staff meets the qualification of program leader or program director.
