During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated completed on 02/24/2026, as a part of the licensure period. The on-site inspection began at 7:25 AM and ended at 8:21 AM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 1 staff. The inspector reviewed 5 children?s records on site and 4 staff records [on-site] OR electronically on March 2, 2026.
This inspection included.:

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March9, 2026. A POC submitted after this date will not appear on the public website.
Violations
13The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
There was no documentation of immunizations for child 3 who began attending care on February 9, 2026 and was in care during the inspection.
The most recent tuberculosis screening provided for staff 4 was not within the last two years.
Staff 1 did not complete the Virginia Department of Education-sponsored orientation course that was required to be completed by February 3, 90 days after employment.
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff 1 stated that she started working with the children in November 2025. There was no documentation that staff 1 had obtained the required orientation training. Staff 1 was alone with the children during the inspection.
Staff 1, alone with the children for 70 minutes during the inspection did not have instruction in performing the daily health observation of children.
Staff 1 was alone with the children from the center opening at 7:00 AM until another staff person arrived at 8:10 AM at which time there were seven children in care.
Staff 1 and staff 2 are regularly scheduled to care for the children in the before school program. There was no documentation to demonstrate that either staff meets the qualification of program leader or program director.
There is no documentation demonstrating program leader qualifications for staff 1 or 2 regularly assigned to care for the children in the before school program. Staff 2 was on duty by herself for the first 70 minutes of the program the day of the inspection.
Staff 1, on duty by herself for the first 70 minutes of the operation of the before school program does not have FA or CPR certification.
Staff 1 did not know the code to access the first aid kit/supplies. First aid supplies were not available for use.
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.
There was no record available for child 2 who began attending care on February 9, 2026.
Staff records shall be kept at the center for each person that includes all required information.
The record for staff 1did not include the hire date, position title or a person to be contacted in an emergency.
The record for staff 2 did not include documentation of two or more reference checks before employment.
Staff 1 does not have CPR certification.
Staff 1 does not have first aid certification.
