During the inspection, the inspector reviewed the areas listed. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated on March 4, 2025, and completed on March 4, 2025. The on-site inspection began at 10:55 a.m. and ended at 1:10 p.m. The inspector reviewed compliance in the areas listed above. There were 31 children present and three staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews and observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 14, 2025. A POC submitted after this date will not appear on the public website.
Violations
3During the inspection, the licensing inspector noticed an unsupervised child (four years old) outside of the playground fence. Staff #2 said the child went inside unsupervised, walked through the building and came out on the opposite side of the building.
The licensing inspector observed another unsupervised child (five years old) walking through the building and took the child back to Staff #3 who was in the restroom in another classroom with a group of children. Staff #3 did not realize that the child had left the room and was unsupervised.
There were 31 children on the playground with three staff from 10:55 a.m. to 11:50 a.m. and no other staff were on site. There were three-year-old children in care, including one 2 year old who had been assigned to the different age group based on parent permission and a written developmental assessment.
Child #2 had emergency medication on site and no parent authorization for the medication.
