Inspector Notes
A monitoring inspection was initiated from 1:35 pm - 2:45 pm to investigate a self reported incident involving care, protection, and guidance, and procedures for emergencies. Interviews were conducted, documentation, and video footage was reviewed. The self reported incident regarding care, protection, and guidance, and procedures for emergencies is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

2Standard 8VAC20-780-340-A
Based on interviews, documentation and video footage it was determined that staff did not always ensure the care, protection and guidance of children while in their care. Evidence: On 5/19/2022 at 2:13 p.m. child A (DOB: 08/16/2021) was seated in an infant high chair. Staff A is observed picking up the high chair by the tray at waist level while the child was still seated in the high chair. The tray snapped off of the high chair causing the child and seat to fall to the ground. The chair hit the ground and the child fell back hitting their head on the floor. Staff A did not report the incident to administration until 4:30 p.m.
Plan of Correction: Employee who failed to use common sense and moved a child without any reason was terminated after the documentation and video was reviewed on 05/19/2022.
Standard 8VAC20-780-550-N
Based on interviews, documentation and video footage it was determined that the center failed to notify parents immediately when a child sustains a serious injury. Evidence: On 05/19/2022 at 2:13 p.m. child A (DOB: 08/16/2021) was seated in a high chair. Staff A can be seen picking up the high chair by the tray. The tray became detached from the high chair causing the chair and the child to hit the ground. The child?s head is seen hitting the floor. The parents were not notified about the incident until 4:30 p.m. by telephone.
Plan of Correction: 100% of staff received a re-orientation on safety procedures, reporting requirements and documentation.
