Inspector Notes

An unannounced monitoring inspection was conduced today with the director from 8:30 am to 9:50 am. There were 30 children present with 6 staff, the ages of the children ranged from 9 months to 4 years. During the walkthrough of the building, the Licensing Inspector was able to observe children doing art work and sensory activities in several classrooms.
FIve staff files and six children's files were reviewed. There were no medications on site at this time. The fire inspection was dated 2/1/23 and the health inspection was dated 1/11/23.

Standard 8VAC20-780-270-A
Based on observation, it was determined that the center was not maintained in a safe condition.
Evidence:
The Licensing Inspector (LI) observed a fan sitting on top of an upper cabinet with the cord hanging down the wall and plugged into the outlet. The cord was hanging and in reach of the children in the Preschool III ( 3 years of age) classroom. If the cord was pulled the fan could've fallen on a child. There were 4 children and 1 staff present in this room during the classroom walkthrough.

Standard 8VAC20-780-330-B
Based on observation, it was determined that there was insufficient resilient surfacing around the play equipment.
Evidence:
1. On the Preschool II and III playground the mulch surrounding the 3 green slides had degraded and turned to soil. Soil is not an acceptable resilient surface.
2. On the PreK/Schoolage playground, there was 4 inches of mulch surrounding the green slide and rock climber. The standards requires at least 6 inches of mulch around these pieces of equipment.
3. On the Infant/Toddler playground there was 1 to 2 inches of mulch surround the blue/yellow slide. The standard requires at least 6 inches of resilient surfacing (mulch).
