8VAC20-780 Staffing and Supervision.

Inspector Notes

An unannounced monitoring inspection was initiated on 5/28/25 and completed on 6/18/25 in response to a self-report received by the licensing office on 5/22/25 relating to staffing and supervision. The inspector reviewed 2 staff records on-site on 5/28/25. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/4/25. A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 5/19/ 25 at approximately 8:50 am, staff A left the toddler classroom unattended for approximately one minute. There were 8 children in the classroom, the youngest child was 21 months of age.
Plan of Correction: This was a self reported incident due to a 45 second gap which was verified on video footage. Staff were immediately corrected and retrained.

The staff to child ratio whenever children are in care is 1 staff to 5 children for children 16 months up to 24 months.

On 5/19/2025 at approximately 8:45 am staff B left the toddler classroom, leaving staff A alone with 8 children for approximately 3 to 5 minutes.
Plan of Correction: Staff members were retrained on ratio and group size requirements.
