An unannounced inspection was initiated and completed on 2/25/26 in response to a complaint received by the licensing office on 2/24/26 relating to allegations of disqualifying background check.
The inspector reviewed one former staff record on site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
