Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 10/25/2021 and concluded on 10/26/2021. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 23 children present, ranging in ages from 3 to 6 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Based on a review of staff records, 1 out of 2 staff members did not have a central registry background check every five years after employment.

Evidence:
1. Staff member #1's (doh: 9/2001) most recent central registry background check on file was dated 10/16/2016.
Plan of Correction: We will send out a new one right away.
