Inspector Notes

Today an unannounced Renewal Inspection took place between the hours of 1:05pm-1:50pm. There were 16 children with 4 staff within the supervision guidelines. The children were observed listening to a story, doing individual activities, having rest time and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency materials were observed during this inspection. Based on a review of staff records, all staff did not have a completed sworn statement or affirmation on file prior to employment.

Evidence:
1. Staff #1 (doh: 8/28/2023) had a sworn statement on file that was dated 9/5/2023. The sworn statement was not filled out prior to employment.
Plan of Correction: Moving forward, we will make sure all staff fill out a sworn statement prior to employment.
