Inspector Notes

A monitoring inspection was conducted; there were 10 children present with three staff supervising. Children were observed at circle time and playing outside.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center did not have the required depth of resilient surfacing.
Evidence:
1. The two slides had grass growing up through the mulch in the fall zone; mulch was approximately 3-4 inches and the required depth is 6 inches.
Plan of Correction: Mulch will be redistributed so the required depth is maintained.

Based on record review, the center did not have proof of I.D.
Evidence:
1. Child record #1, #4 and #5 die not have proof of ID.
Plan of Correction: A letter will be submitted to the Buchanan County Sheriff's office and a copy of the letter will be sent to the inspector.
