Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A (monitoring/renewal/complaint) inspection was conducted on 2/21/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 27 school aged children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based upon review of staff records, the facility has not ensured that results of a fingerprinting/criminal record is obtained prior to the first date of employ.
Evidence::
The record provided for staff 2 indicated a date of employment of 6/20/21 and first day supervising children of 6/21/21. The results of fingerprinting were not obtained until 7/19/2021.
Plan of Correction: The facility responded with the following:
In the future we will ensure that results of fingerprinting are obtained prior to the first date of employment of staff.

Based upon review of two children's records, the facility has not ensured that a statement from each parent advising of their child's swimming skills.
Evidence:
1. The record provided for child 1 did not include a statement from the parent advising of the child's swimming skills.
2. The record provided for child 2 did not include a statement from the parent advising of the child's swimming skills.
Plan of Correction: The facility responded with the following:
Parents will be required to provide a written statement advising of their child's swimming skills.
