Inspector Notes

An unannounced renewal inspection was conducted on 04/04/24 from 10:40am to 11:45am. At the time of the inspection there were 43 children in care and 7 staff members. Children were observed participating in various activities including center play, circle time, and arts and crafts. Records were reviewed for four children and three staff members. Evacuation drills, emergency supplies and other required records and postings were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed with the director during the exit interview.

Based on observation and review It was determined that the center has not ensured that staff and children's records are treated confidentially.
Evidence:
1.The site information display board had the Inspection Supplemental sheet posted on the parent board. The Supplemental sheet includes the confidential first and last names of the staff and children observed or records reviewed during the inspection. The Supplemental Information sheet specifically indicates that the sheet is not to be posted at the facility.
2.Staff #1 (Director) confirmed that the supplemental page was posted on the site information board.

Based upon review of children?s records, it was determined that the center has not ensured that they maintain a separate record for each child enrolled that includes all required information.
Evidence:
1.The record for Child #1 was missing a work phone number for both parents.
2.Staff #1 (Director) confirmed that the work numbers were missing from the record.
