Inspector Notes

An unannounced monitoring inspection was conducted on October 23, 2023 from approximately 12:50 pm to 3:00 pm. Sixteen staff were present supervising fifty-eight children ranging in age from infants through preschool. Additional staff were on site for administrative duties. Children were observed playing outside and napping. Five child and five staff records were reviewed. Violation found is noted on the violation notice.

Based on observation, facility did not ensure there is resilient surfacing in fall zones at least 6' around the perimeter of play equipment that is climbing and/or has moving parts.

Evidence: On the infant playground, there is 2' - 2' 6" of fall zone around the perimeter of the slide play structure between the structure and the plastic border and between the structure and the post for the shade tent covering the playground.
Plan of Correction: The slide play structure will not be used until needed corrections are made.
