Inspector Notes
A complaint investigation was conducted in conjunction with local DSS agency concerning care of children. Interviews with staff, records for staff and children as well as attendance documentation were all reviewed. Based on the preponderance of evidence, the allegation is founded. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 2Evidence:
On 1/4/2023 the ratio of 1 caregiver to 5 children was exceeded for approximately 20-25 minutes as 6 children were present from 9:30-9:55 am with Teacher #1. A second Teacher (#2) entered the Monkeys classroom at approximately 9:55 am but left after a time of approximately 5-10 minutes before Teacher #3 entered the classroom and stayed for the remainder of the day.
Evidence:
Child #2 left the monkey classroom when the LT left at 3:00 p.m. and was taken into the center director's office with a staff member (Teacher #4) until pick-up at approximately 5:22 p.m. The office is not child proofed and is not regularly used for children's activities. The child was moved rather than the staff member to accommodate a later pickup time.
