Inspector Notes

A complaint investigation was conducted in conjunction with local DSS agency concerning care of children. Interviews with staff, records for staff and children as well as attendance documentation were all reviewed. Based on the preponderance of evidence, the allegation is founded. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-350-B-2
Based on staff statements and attendance records, the ratio for children ages 16 months to 24 months was not maintained in the Monkey's classroom.
Evidence:
On 1/4/2023 the ratio of 1 caregiver to 5 children was exceeded for approximately 20-25 minutes as 6 children were present from 9:30-9:55 am with Teacher #1. A second Teacher (#2) entered the Monkeys classroom at approximately 9:55 am but left after a time of approximately 5-10 minutes before Teacher #3 entered the classroom and stayed for the remainder of the day.
Plan of Correction: I will make sure that in future all the classrooms maintain the classroom coverage at all times.
Standard 8VAC20-780-360-A
Based on staff statements, attendance record and confirmation by center administration, the activities for a child were not age and stage appropriate.
Evidence:
Child #2 left the monkey classroom when the LT left at 3:00 p.m. and was taken into the center director's office with a staff member (Teacher #4) until pick-up at approximately 5:22 p.m. The office is not child proofed and is not regularly used for children's activities. The child was moved rather than the staff member to accommodate a later pickup time.
Plan of Correction: In the future, no child will be taken to the CM office. I will make sure that the child will stay in the class room with a teacher.
